学生健康保险

The 学生健康保险 Premium is assessed to students registered in most academic units on campus (excluding University College 和 项目 including the Graduate Tax, Executive MBA 和 Continuous Enrollment for all 项目). The premium is assessed each fall 和 spring term for students enrolled in one or more credit hours per quarter (one or more credit hours per semester). The premium is included on the tuition bill 和 covers a six-month period.

Students whose first term for an academic year is either winter or summer term may opt into the 学生健康保险 Program (SHIP) by contacting 学生健康服务 to obtain the necessary application forms 和 information.

  • 如何参与

    If you choose to participate in the 学生健康保险 Program, complete the form located in MyDU.

    1. In MyDU:选择以下选项:

    2. Once on the Health 和 咨询服务费用 Waiver page, students simply need to click the radio button to elect coverage. An email confirmation will be sent within 48 hours confirming the coverage choice.
    3. Even if students accept coverage in the 学生健康保险 Plan, they must meet the minimum enrollment requirements. If a student accepts coverage 和 is enrolled for 1 or more hours the student’s acceptance will be communicated to the insurance carrier. If you have specific questions regarding the plan benefits or coverage, please contact 学生健康服务 at (303)-871-2205.

Students may waive participation in the program 和 the insurance premium provided they are covered under another adequate health insurance policy. 作为豁免程序的一部分, students will be asked to provide policy information for their personal coverage.

An email confirmation will be sent to you within 48 hours. Keep a copy of the confirmation for your records.

    4. If a student’s course registration meets the requirements for automatic enrollment in the student health insurance program (see top paragraph), 和 the student does not waive participation in the program by the stated deadline, the student will be automatically enrolled in the student health insurance program 和 the health insurance fee will be charged to the student's account.

The 学生健康 和 Counseling fee is assessed each academic term for students registered in one or more credits hours. This fee is m和atory for undergraduate students, but may be waived by graduate students each term by completing the waiver form located in MyDU

Currently registered students who have not been automatically assessed the health 和 counseling fee upon registration may enroll in the plan by contacting 学生健康服务 at (303) 871-2205.
